Use the link below to access the Jotform portal, where you can upload new or updated required documents and complete an updated Accounting Memo digitally.

The link below will take you to a Jotform portal to complete your vendor registration. Required fields are marked with an asterisk (*), and the form cannot be submitted until all required information is completed. The registration form collects all information requested in the Accounting Memo, so a separate memo upload is not required.
In addition, submission of the vendor documents listed below is required. Sample documents are available at the Google Drive link below. Final, completed documents may be submitted via the online Jotform or by email to vendordocs@grindertaber.com.

• W9 (blank form accessible by link above, signed in current calendar year)
• Any MWBE Certificates (if applicable)
• Certificate of Insurance* (blank form and requirements accessible by link above)
*Please note that you may send over “Evidence of Insurance” for the time being. If you are awarded a project, then the COI must be brought into full compliance.
